Hyperbaric oxygen therapy is a widely used and generally safe treatment, but there are contraindications that create risks for certain individuals. To ensure your safety, protect your medical identity, and verify your coverage, we require all patients to complete our intake process before an appointment can be scheduled.
What we will need from you:
Valid government issued driver's license or id card;
Current insurance card(s);
Completed patient consent and intake forms.

[Please note: If we are in network with your insurance carrier, we will need to check eligibility and notify them of our intent to treat prior to scheduling your first appointment. This is required for every in-network patient and does not indicate that you have a covered condition.]
